General Description[]
Isaac Smith was a highly respected Doctor for Pillbox Medical Center before retiring and leaving the city a couple years back.
On the 29th of October, 2022, Isaac made a sudden appearance back in the city. Similar to his brother, he carried a supernatural entity within him - Pestilence, the first Horseman of the Apocalypse.
Isaac would remain unaware of the evil that lurked inside of him up until the time of his death. On the 4th of January, 2023, Pestilence would end Isaac's life and take full control over his body.

Isaac[]
Isaac used to be seen as cold or harsh, to a lot of people around him. After returning to the city he seemed to be more regretful of his past self and has apologized to multiple people for his past behavior. During conversations he was often polite and cordial, though people who have heard of what he might have become since his disappearance are cautious to what his true feelings might be.
Isaac was seemingly unaware of when Pestilence would take over his body and would forget the moments it acted without him. Norman revealed that Isaac had issues with his memories and didn't remember much of his past, including Norman.
Pestilence[]
As Pestilence shows itself Isaac's voice becomes more unhinged. He likes to use endearing terms weaved in with maniacal laughter to unsettle those he speaks to. He's not hesitant to make threats and to let people know what is coming for them.
Due to his unpredictable nature, Pestilence is arguably one of the most dangerous horsemen, something that even Norman has prefaced in conversation.
Isaac is a caucasian man with slicked back light grey hair and longer beard. He wears glasses and is usually seen wearing classy clothes in white. Whenever Pestilence would take over his body he would often wear a black plague doctor mask and all black.
He has a pretty soft spoken deeper voice and British accent that can make him come off as rather posh. As Pestilence his voice can best be described as creepy and utterly unhinged.
The full extent of Pestilence's powers is yet to be revealed but some things are known so far.
- He is able to make people sick and create plagues. Several people have been seen throwing up during events believed to be related to Isaac. He made Pixie Plum vomit by merely touching her shoulder. Pestilence's involvement has also been attributed to Richard Dark's declining health where his blood has been turning a black color and he's been affected by a heavy cough.
- Live rats have been found inside the stomach of Dark as well as inside of Flop, both believed to be the doing of Pestilence.
- According to Norman, the suffering Pestilence is capable of bringing to the world would make people beg for him to take them instead.
- He has the ability to keep the person he's inhabiting from dying.
Background[]
To outsiders, not much is known about Isaac's past before he moved to Los Santos, shortly after his wife passed away in Liberty City. He found it too difficult and haunting to continue living and working in the city where she died.
Isaac and his brother Norman Bones were born and raised in Chicago, Alaska. Throughout their younger years of living with their parents, they were abused both mentally and physically. When the brothers were old enough to plan their escape from the abuse, they took action, in the form of killing their parents. Isaac was tasked with killing their mother (due physical stature/age), and Norman killed their father.
The brothers parted ways shortly thereafter, and led separate lives for several decades with no communication. Sometime during this period, Isaac went on to study medicine, and became a hematologist in Liberty City. In January 2019, shortly after his brother began work in Los Santos, the two brothers were reunited at Pillbox Medical during one of Norman's visits.
Relationship with Brother[]
The relationship between Norman and Isaac is not well-known in Los Santos as they only ever met in private in order to keep their relationship a secret.
The brothers share a deep bond, perhaps brought about by the fact that they murdered their parents together. Isaac viewed his brother, a known serial killer, as Death, even before he died and killed the previous incarnation of Death.
One day when Isaac was contemplating his own death, he told Norman, who was still human at the time, when the time came he wanted Norman there to guide him to the other side, specifically referencing The Lighthouse. "If I'm going to die, I want you guiding me to the light."
Return to Los Santos[]
Isaac first appeared back in Los Santos after Flop, Dark and Rhodes brought up his name to Norman as a way to get out of a deal. Norman made it clear that even going as far as mentioning his brother was a mistake on their behalf.
Shortly after, Norman met up with his brother and let him know that his name has been circling around the city and that one family in particular even went so far as to try and bargain with it. Although the family was still tied up in a deal with Norman, he allowed Isaac to do whatever he wished with the family once their deal was done.
Before their deal was up however, Isaac found an opportunity to kidnap[1] an injured Flop and was hoping to ask him some questions. Unfortunately however, Jackie Snow spotted the car they were in an pursued them. Unable to get away from the police, Isaac eventually let Flop go and nothing else came from the situation.
Isaac also made some in person appearances, mostly catching up with people who used to know him back in the day. He explained his reason for returning as checking in because he heard that Pillbox, his old working place, had been shut down.
It is unknown how long he plans on staying in the city but through multiple unknown phone calls Pestilence has let it know that the plague is coming along with the other riders (War, Famine and Death).
Isaac's Death[]
According to Norman, his brother had been dead for a long time. The only reason why he was still in this world was due to Pestilence keeping his body together and denying him his death.
On the 4th of January, 2023, Pestilence decided it had had enough of Isaac and would end his life.
Isaac had a risky nerve surgery planned for the day that could end up killing him.
Once Isaac arrived at the hospital and was in the operating room, Pestilence took over his body. It made him stab Harlow Mathews, the doctor who was in the room with him. Hearing her screams, Olivia Copper came into the room where Pestilence demanded that she shoot Isaac.
She refused, saying that she'd never give Pestilence what it wants. Pestilence responded that if it couldn't have him, then no one could. Isaac collapsed on the floor and his body started to rot away and pestilence began to spread affecting those who where there. Norman stepped into the room shortly after to have one last look at what remained of his younger brother.
After his passing, Isaac would find himself in Purgatory where he'd meet his brother once again. Isaac, who until this point had forgotten most of his childhood together his brother Norman, would end up remembering all the awful things that happened to them as children.
Norman would apologize for all the things Isaac had to endure up until that point. They shared a moment of grief before Norman had to leave. He promised that they would meet again before facing oblivion.
Pestilence would remain in control over Isaac's body after his death. They would retire while gathering new strength for the apocalypse that is to come.

- It has been said that he has an infatuation with the human body.
- He is believed to be one of the Four Horsemen of the Apocalypse (known as Pestilence, War, Famine and Death).
- Similar to Norman's crows - Isaac has rats to help do his bidding.
- Andi Jones once likened Isaac and Pestilence to a combination McDonald's and Diary Queen.
